How do I re-encode MKV to M4V without losing video quality?

Both MKV and M4V are modern containers — which should I actually keep?

Keep whichever matches where the file is going, because neither is better in the abstract. One of these two is built for playback compatibility and the other for holding everything a file might contain, and they trade against each other exactly along that line. The pragmatic answer for most libraries is to archive in the feature-rich container and export to the compatible one on demand. MKV is the archival container of choice because it stores unlimited audio tracks, soft subtitles, chapter markers and cover art in one file. M4V keeps chapter markers, artwork and subtitle tracks in the shape the Apple TV app expects, which plain MP4 often loses.
Which video codec does the M4V output use?

It depends on the M4V container. MP4 defaults to H.264 for the broadest playback support across phones, browsers, and Smart TVs. MKV defaults to H.265 / HEVC for roughly half the file size at equivalent quality. WebM defaults to VP9. The advanced options expose explicit codec choice if you need to override. Will my audio track survive MKV to M4V conversion?

Why is the M4V file blurry compared to the MKV source?

Almost always a bitrate-too-low setting. Re-encoding from a high-bitrate MKV into a default-CRF M4V compresses heavily on motion-rich scenes. Push CRF down to 16-18, or set an explicit target bitrate matched to the MKV source, and the M4V regains its clarity.
